What is the standard for lactation breaks for employees in your practice?

Of the nearly 3000 VINners taking last week's Quick Poll more than one third (36.89%) said they prefer to be addressed in the most formal way, Dr. "Last Name", exclusively. Only about 10% are on a first name basis with both clients and staff.
